Review notice on MediaWiki account creation on top of the page. #133

Notice on top of page warns where to tell if a problem occurred wile creating a wiki account.

Maybe we should warn that this account is only about the docs service and not the annotation server and that accounts.webplatform.org is going to have all accounts in a foreseeable future.
