The reviewer has been wondering about the cause of medium-speed scenarios being more cost-effective than fast-speed scenarios. We initially speculated that this is a result of the marginal ICER as vaccines move on to younger populations. We provided evidence to show that. See also here.

I think additionally, at least for earlier programme start dates, that fast-speed scenarios, compared to medium speed ones, will deliver more doses after the epidemic wave to those who have already been infected/are at lower risk.
