• Add a portal that spawns underneath the raft.
• Bail on trees and other obstacles. (area must be clear from ship to ground for the portal to work)
• Make the portal a pool of light that must be jumped into to trigger.
• Jump action triggers in and out of the portal.
• For air vehicles
• Portal locks the player in position until the boat is rendered.

Fix other portal problems if applicable during this work. Including height map issues and water levels being out of sync and drowning player.

Like it. Name could cause some confusion though - "TargetPortal" is an existing mod that allows you to name vanilla portals and select (from the map) which one to exit as you enter them. "Portal Ladder"? "Ground Portal"? Those sorts of portal mods also introduce a mechanic that may mess with this sort of setup if it's going to locate two portals on essentially the same XY co-ordinates, making selecting a desired destination impossible if it's using standard portals that those alter.
